Key Responsibilities :

  • Conduct in-home assessments and create personalized physical therapy plans for patients.
  • Provide one-on-one physical therapy sessions, tailored to individual patient needs.
  • Collaborate with healthcare professionals, including nurses and physicians, to deliver comprehensive patient care.
  • Educate patients and their families on exercises and techniques for ongoing care.
  • Document patient progress and maintain accurate records.

    • Qualifications :

  • Valid Physical Therapist license in the state of Illinois.
  • Strong clinical skills and the ability to provide high-quality patient care.
  • Compassion, empathy, and excellent communication skills.
  • Reliable transportation and a willingness to travel to patients' homes.
  • Prior experience in home health care is a plus, but not required.
