Monitor the health conditions of individuals served and ensure quality services providedProvide assistance and / or supervision to individuals in the areas of daily living, personal care, and community / social integrationHelp maintain safe and quality supports for individuals servedInitiate emergency procedures on a needed basisUse equipment correctly and safelyReport any hazardous conditions, equipment, accidents, or incidents to supervisorMaintain current progress notes and any other appropriate documentationAssist with resident activity programs, including off-site outings and off-site appointmentsAssist with household activitiesActively attend and participate in team meetings and training as requiredMaintain confidentiality and treat others with dignity and respect in all you doQualifications : High School diploma or G.
