- Under the direct supervision of the Chief Medical Officer, the Family Nurse Practitioner is responsible for completing physical examinations, diagnosis, and treatment of both chronic and episodic disorders to assigned clients;
- plans, implements, reviews, and confirms health care goals for assigned clients; remains responsible for the client(s) throughout the course of the treatment;
determines when the client has recovered, and recommends release of the client. May refer the client to physicians for consultation or to specialized health resources for treatment but retains responsibility for clinical management as well as any subsequent modification.
MINIMUM QUALIFICATIONS REQUIRED FOR THIS POSITION :
Graduate from an accredited School of Nursing.
A current license to practice as a Registered Nurse and a certificate to practice as a Nurse Practitioner issued by the California Board of Registered Nursing.
Prescriptive Privileges and active and current DEA
A current CPR Certification required.
At least one year’s experience working in family medicine.
Baccalaureate or Master’s Degree in Nursing with emphasis in Family Nurse Program can be
substituted for one or two years' experience, respectively.
National Board certification from any recognized Credentialing Center pending or currently in
process.
Experience and ability to work with multi-cultured, multiethnic populations.
Visual / hearing ability sufficient to comprehend written / verbal communications.
Ability to perform tasks involving physical activity which may include lifting, bending and
standing.
The ability to work additional hours, days and / or on special projects as needed.
Ability to effectively deal with stress
GENERAL STATEMENT OF FUNCTIONS :
Develops treatment plans for acute and chronic diseases
Performs complete physical examinations, including interviewing clients to obtain complete medical histories, social, psychological, family histories as well as performing, ordering, interpreting and evaluating laboratory procedures, diagnostic tests and examinations.
Diagnoses and treats both acute, chronic and episodic disorders including complications of otherwise routine processes.
Establishes and documents the health care plan and prognosis, maintaining records of each case that are detailed and complete for any physician reviewing them to evaluate the effectiveness of the plan.
Initiates and furnishes drug therapy, taking responsibility for other reactions as well as therapy when performed prior to consultation with a physician.
Determines when the client has recovered from the disorder and recommends the release of client from treatment. Maintain accurate and current clinical documentation of care given and clients response and
progress to treatment.
Educates and guides patients on disease prevention, disease management, and healthy
lifestyles habits
Prepare reports and other verbal and written documentation required by Agency.
Provides in-service education classes on assigned responsibilities for medical assistants and
other nursing staff
Attend and participate in Case Review Conference (CRC), staff meeting, educational and
training sessions.
Maintain current licensure, certification and other professional development necessary to
provide a high standard of clinical practice at all times.
Practice confidentiality in all client, employee, and management related matters.
Assist in preserving Kedren’s mission, goals, and objectives; and help management in
establishing its internal service delivery goals and objectives that are consistent with the
organization’s public service mission statement.
Participate in the fundraising activities of Kedren.
Assist in other duties and special projects as assigned.
