JOBSUMMARY :
Hybrid 2 days in office 3days from home after new hire orientationperiod.
Provides social workservices including asessment care planning and case coordination topatients and their families. Provides consultation to staff andother health care providers.
Initiates social work plan of care.Serves as liaison between Kaiser Permanente and community tointegrate services coordinate care for clients and enhancerelationships through public relationsactivities.
REQUIRED KNOWLEDGE ANDSKILLS :
one (1) year social workexperience in hospital medical setting home health etc. Experiencewith advanced care directives.
Experience in insurancereferrals.

- Licensed Social Worker(Hawaii) OR Licensed Clinical Social Worker (Hawaii)
- Basic Life Support required at hire fromAmerican Heart Association Healthcare experience working withmedical diagnoses.
- Demonstrated ability innetworking with communityagencies.
