About The Position
Job Title : Registered NurseJob Specialty : Home Health Job Duration : 260 days Shift : 8 Hours Guaranteed Hours : Not specified Experience : Minimum of two years in a home setting within the last ten years License : Current RN Licensure in New York State Certifications : BLS (required before the start date) Must-Have : - Recent experience conducting comprehensive assessments of patients- Experience in a home setting within the last ten years, such as working for a Certified Home Health Agency (CHHA), Licensed Home Care Service Agency (LHCSA), or Managed Long-Term Care (MLTC)- Experience providing primary care to homebound persons- Care management experience for persons receiving home and community-based services, including Hospice and Assisted LivingJob Description : - Perform initial in-home pre-evaluation of patients, including evaluating health status, strengths, care needs, and preferences, and guide the development of individualized long-term care service plans- Review consumer medical documentation and health referral forms as relevant to the case- Enter evaluation data into an electronic evaluation form and transmit as required- Document any concerns, conflicting information, or issues that surface during the evaluation process- Emphasize continuity of care to reduce or eliminate fragmentation, duplication, and gaps in the treatment plan- Discuss healthcare options, necessary supports, service vendor options, and waiver options with consumers- Initiate communication with eligible consumers for enrollment into a managed care health plan- Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery- Collect quality review data and required documentation to support outcome measurements and record case notes- Identify opportunities for health promotion and illness prevention- Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population- Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template- Perform other duties as assigned by the Regional Nurse Manager or project management
