TheNurse Practitioner / Physician Assistant leads the medical aspects ofteambased care for individuals with serious mental illness that weserve.

This position is responsible for documenting and supportingthe medical needs of the individual by performing a wholepersonclinical assessment to identify and remediate care gaps.

On anongoing basis this position will support care coordination amongthe individual s medical and behavioral health providers makingreferrals as appropriate as well as provide clinical support toGuides (peers) and Community Resources Guides (socialworkers).

Responsibilities

Performa wholeperson clinical assessmentincluding :

  • Gatheringinformation from outside sources (especially other careproviders)
  • Assessingboth behavioral and physical health conditions through a thoroughhistory and physicalexamination
  • Capturingconditions not yet diagnosed including by ordering additionalstudies asindicated
  • Organizingthe information in a manner that supports successful disabilityapplications (ifappropriate)
  • Conductingvisits in the field or in an individual s home whenneeded
  • Makingindividualized recommendations and / or referrals for programs / careprovided by the company or outside providers based on the assessedneed

Leadthe medical component of the teambased care providedincluding :

  • Trainingand advising Guides and Community Resources Guides on clinicalissues andtreatments
  • SupportingGuide Team on carecoordination
  • Leadingcaremanagement
  • Identifyingactionable caregaps
  • Performingmedication reconciliation postdischarge and asneeded
  • Communicatingand coordinating with individuals care providers primarily at CMHCsFQHCs andhospitals

Theexperience you bring to this roleincludes :

  • 5years clinical care as a Nurse Practitioner / Physician Assistantideally in a primary caresetting
  • Workingwith and managing underserved populations with challenges such asbehavioral health and / or SUDconditions
  • Developingand delivering educational programs to nurses and affiliatedmedicalstaff
  • Caremanagement andcoordination
  • HCCCoding

5+ years clinical care as a Nurse Practitioner / Physician Assistant,ideally in a primary care setting. (Must be licensed in the stateof Washington).

Working with and managing underserved populationswith challenges such as behavioral health and / or SUD conditionsDeveloping and delivering educational programs to nurses andaffiliated medical staff Care management and coordination HCCCoding
