What You’ll Do

Provide the full scope of primary care services, from preventive and chronic disease management to acute care.

Collaborate with a multidisciplinary care team to address medical, behavioral, and social determinants of health.

Document and manage patient care using EPIC EHR, meeting quality and timeliness standards.

Deliver culturally competent care in a diverse urban community.

Serve as a mentor and educator to trainees, residents, and future health professionals.

What We’re Looking For

MD or DO with completed residency in Family Medicine or Internal Medicine

Board Certification in primary care specialty

Active Illinois medical license (or ability to obtain)

Commitment to the mission of equitable, community-based health care

EPIC experience preferred, but not required
