Job Description
All About Home Care is looking for a reliable and professional Occupational Therapist (OT) who is skilled in providing therapy in patients’ homes. We pride ourselves on hiring professionals who understand the unique dynamics of Home Health care.
Qualification
Graduate of an Occupational Therapy curriculum accredited jointly by the committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and / or certified by successfully completing the national Certification examinationCurrently licensed in the State of TexasTwo years of experience in Home Health preferred1 year experience using Kinnser / WellSky for home healthResponsibilities
OT takes initial history and make initial evaluationAssists the physician in evaluating level of functionImproves or minimizes residual physical disabilities of the patientReturns the individual to optimum and productive living within the patient’s capabilitiesProvides prescribed occupational therapyDirects and supervises personnel as requiredPerforms all skilled proceduresConsults with physicians regarding change in treatmentWrites reports to physicians regarding patient’s progressInstructs patient / family members in home programs and fine motor development exercisesAn initial evaluation, including plan of treatment and goals, must be filled out and submitted to the physician for approval. A progress note is written each visit. A re-evaluation is conducted when expected duration of treatment is reached. A summary is written upon patient’s discharge.Helps develop the plan of care and revises as necessaryPrepares clinical and progress notesAdvises and consults with the family and other Agency personnelPeriodically participates with all other health care personnel in patient care planningParticipates in in-service program
