About Us
Lāna'i Community Health Center (LCHC), located on the island of Lāna'i, is deeply rooted in a unique history of transitions and cultural diversity.

LCHC serves the entire population of Lāna'i, with a special emphasis on those living at or below 200% of the federal poverty level and the underinsured or uninsured.
The center provides services in a culturally sensitive manner, offering translation services when needed, and ensures that no one is denied care due to an inability to pay.

Role Overview
The Director of Quality and Risk Manager (DQRM) is responsible for the development, implementation, and evaluation of an overall plan for achieving a culture of quality and patient safety.
This position is also responsible for assembling and providing informatics as needed for quality and other programs. The DQRM will focus on Risk Management, Privacy and Safety Officer duties, as well as participating in clinical programs and health education activities.
This individual is also involved with program planning, organization, operations, and staffing as part of the LCHC Leadership Team.
The DQRM will assist with daily coordination, implementation, monitoring and reporting on the center's Quality Improvement and Risk Management programs.
Integral member of the LCHC Leadership team; Chair of the LCHC Quality Assurance Committee; works with Leadership to provide clinical staff with continuing education opportunities, direct clinical training, training supervision and evaluation of skills learned.
Requirements
Risk Management, Safety, and Privacy - 60%
- Oversee and coordinate the health center's risk management activities, including completion of quarterly risk management assessments, provision of risk management training to health center staff, submission of an annual risk management report to the Board, and other tasks as described in the health center's Risk Management and Patient Safety Policy.
- Providing oversight of risk management and patient safety actions and reporting; participate in compliance program activities;
taking minutes in the monthly quality meetings and preparing necessary presentations.
Completes LCHC annual risk assessment and action plan, quarterly risk management assessments, HIPAA risk assessments no less frequently than every three year and more frequently if changes in technology or cyber risks require, provides risk management training to health center staff, submission of annual risk management report to the LCHC Board and Leadership team and other tasks.
Director of Quality - 40%
Implements and leads a continuous quality assurance and improvement process throughout the program and service areas, focusing on systems and process improvement.
Promotes regular and ongoing opportunities for all staff to give feedback on program operations.
- Develops, implements, and evaluates an overall plan for achieving a culture of quality and patient safety.
- Assists with the development of strategies for selecting quality measures that support the provision of quality patient care.
Qualifications
- At least a Bachelor's degree in Medical Sciences, Public Health, Public Administration or a related field
- Clinical certification preferred or experience in a clinical or allied health field
- Minimum of three years' experience in health information, quality, utilization or risk management

Salary
- $72,000 - $85,000 annually
