Provides administrative coordination of supports and services to people with developmental disabilities.Plans, develops, and implements strategies designed to promote positive outcomes for individuals.Assists individuals by identifying, accessing, authorizing, and monitoring the utilization of all available financial resources to fund supports in the most fiscally responsible manner possible and in accordance with all Medicaid and DODD guidelines and rules as well as Board policy.Completes all required documentation for needed supports and services, including Medicaid and Medicaid Managed Care programs.Works cooperatively with individuals, families / guardians (as applicable), providers, and other stakeholders to monitor services.Participate in Major / Unusual Incidents (MUI / UI) reporting, investigation, and follow-up, including identifying trends and patterns.Regularly travels to various locations throughout the county for the purpose of service planning and monitoring in a safe, efficient, and effective manner in accordance with agency procedures.Equal Opportunity Employer
