Job Description

The Licensed Practical Nurse (LPN) is responsible for helping assure that all Individuals with developmental disabilities at KenCCID group homes receive timely and professional nursing services.

This professional is responsible for routine home visits, in-office nursing care, and training staff in supporting the health care of Individuals.

This professional is responsible for promoting KenCCID’s vision, mission and values.

The LPN position is a full or part-time non-exempt hourly position, which can include evening, weekend and on-call hours.

This position reports to the Nursing Director.

Job Responsibilities

  • Ensures that all Individuals are being treated with respect and dignity and that the rights of the Individuals are observed at all times
  • Responsible for ensuring that all health care records for Individuals are in compliance with the licensing requirements set forth in Pennsylvania law (55 Pa.

Code 6400, hereinafter 6400 regulations )

  • Utilizes the Health Risk Screening Tool (HRST), pursuant to Pennsylvania Department of Human Services, Office of Developmental Programs (ODP) regulations, to evaluate potential health issues for Individuals
  • Uses HRST as a basis for providing safe and therapeutic care to Individuals
  • Accepts accountability for clinical care of Individuals
  • Train staff on Individuals’ care, coinciding with the assessment, medical history, ISP, and the recommendations in the clinical summary of the HRST
  • Make home visits routinely and as needed to follow up on all the individuals’ physicians / specialists orders and ensure the HRST is updated
  • Monitor and review medications, medication administration records and medical equipment for proper use, documentation and function.
  • Communicate, inform and educate the Residential Directors, House Managers, Program Specialists and Direct Support Professionals regarding any medical / psychiatric recommendation from the HRST Team in reference to the Individual’s care via channels including email and copy the Director of Nursing and other relevant team members.
  • Be proficient with the Individuals ISP and know how it relates to the HRST to meet the Individuals’ optimum needs and making recommendations to the Program Specialists and Residential Directors for ISP revisions
  • Act as a liaison, in collaboration with the Director of Nursing between KenCCID, the HRST Team, the HRST gatekeepers at PCHC and any external entity participating in the care of the Individuals
  • Attend all ISP meetings and other team meetings that require input from the nursing / HRST team
  • Be knowledgeable of the 6400, 6100 and Appendix K regulations and all care regulations and ODP bulletins and assure compliance
  • Update changes in the HRST system as it occurs in the Individuals’ medical, psychiatric and behavioral health no more than 48 to 72 hours after the change occurs
  • Monitor and make recommendations to assure proper utilization of MY 25 Meals plan and other dietary plans in place
  • Monitor and assure accuracy in all medical reports
  • Ensure that all medical reports are scanned and documented in the electronic health records (EHR) database, including but not limited to medical history summaries, medical visits, nurses’ notes, summary of medical discussions with families, support coordinators, and other stake holders
  • Participate in a weekly rotation of on-call nursing duties
  • Document purpose of calls and resolution in EHR
  • Attend medical procedures and appointments for Individuals as needed and assure discharge instructions are implemented
  • Develop and or update medical history summaries as per regulations.
  • Participate in new Individual intake process by making recommendations to the teams for a medical compliance intake
  • Assumes responsibility for professional growth by current reading and attendance at educational programs and unit conferences

Minimum Education and Experience Requirements

  • Current license to practice nursing in the state
  • Minimum of six months of recent experience as a Licensed Nurse Practitioner
  • Experience working with Individuals with developmental disabilities preferred
  • Valid Driver’s License
  • Basic technology literacy, including, but not limited to the ability to read and respond to email, generate reports and access online resources
  • Ability to use online records technology, including scanning and uploading documents to a database
  • Able to effectively multitask

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
  • Prolonged periods of standing, bending, and reaching
  • Must be able to lift up to 50 pounds at times
