Compensation : $225,000 – $250,000 annually

Position Summary :

We’re seeking a dedicated Primary Care Physician (MD or DO) to deliver on-site, relationship-driven care to aging adults in senior living communities. This is a high-autonomy, high-impact opportunity to truly connect with patients and lead care in settings where you’re needed most.

Key Responsibilities :

• Conduct regular rounding visits in assisted living and memory care environments
• Perform comprehensive medical assessments and manage complex conditions
• Develop personalized care plans in collaboration with patients and families
• Work closely with NPs, PAs, and care teams to ensure seamless coordination
• Educate residents and their loved ones about care options and goals
• Proactively manage chronic conditions to reduce hospital admissions
