Pediatric Phone Triage LPN - West Cobb

Established West Cobb pediatric practice seeks an LPN with a minimum of 1 year clinical Pediatric practice experience to perform triage service for Pediatric patients over the phone in the practice office.

This is not a remote position. This individual will serve as the primary contact for medical care to phone-in patients. The Triage Nurse will be responsible for completing an accurate assessment of a patient’s needs, be able to identify symptoms, determine emergencies and if physician interaction is required.

The ideal candidate will have 1-5 years of experience with superb customer service skills, empathy, patience and the ability to reassure and calm concerned callers.

We seek an individual who possesses a broad knowledge of medical terminology and drug formularies, exhibits excellent verbal and written communication skills with a strong attention to detail as well as the ability to work independently.

Prior experience with Clear Triage software is a plus.

Must hold a current Georgia LPN license and be a graduate of an accredited school of nursing

Job Hours (on Site) - Monday- Friday, 9am to 5 pm (lunch daily from 11 : 45- 12 : 45)

Plus 1-2 Saturdays per month (8 : 00am - 12pm)

Salary Commensurate With Experience (plus an extra $4 per hour Saturday Pay)

Key Responsibilities :

  • Respond promptly to each incoming call, portal messages, and provider messages
  • Conduct telephone assessments of patients presenting with medical concerns and

symptoms, utilizing established protocols and guidelines, including those outlined in Clear

Triage, to determine the appropriate level of care required to safely meet the patient’s medical needs (Emergent, urgent, non-urgent, or home care status).

  • Provide medical advice, instructions, and recommendations to patients and caregivers based on assessment findings, ensuring a compassionate and patient-centered approach
  • Refer patients to appropriate medical facilities or providers for care, utilizing approved

guidelines and protocols.

  • Review documents in a timely manner
  • Document all patient interactions accurately in the electronic medical records (EMR) system, ensuring that every instance of providing medical advice is clearly documented.

Maintain detailed records of patient assessments, recommendations, and any instructions given to patients or caregivers to ensure comprehensive and accurate documentation of patient care

Prioritize patient care by checking checkup and Breathewell status before consulting

providers, unless in a life-threatening situation.

  • Assess refill requests from pharmacies, verifying necessity before forwarding to the provider
  • For antibiotic or asthma medication refills, call the patient to understand the situation. If sick, recommend an appointment
  • Coordinate refill requests with upcoming Breathewell appointments for optimal efficiency
  • Be vigilant for signs of distress and escalate as necessary, ensuring patients receive timely care
  • Collaborate effectively with the healthcare team to deliver high-quality patient care
  • Recall Management : Work on recall tasks between patient appointments to ensure follow-up care and continuity of treatment for patients
  • Backup Coverage : Coverage will be needed for the ADHD Behavioral Department and Forms Department on occasion

Kids Health First is an Association of 37 Independent Pediatric Practices located in Atlanta and surrounding areas. Initial candidate screenings are conducted by our recruiter.

All interviews and final decisions are made by the pediatric practices, based on their current individual needs and hiring processes.

Salary and benefits are determined by individual practices.
