At Houston Methodist the Physical TherapistII (PT II) position is responsible position is responsible forevaluating and developing the therapy plan of care for a broadrange of diagnoses and acuities.

This position may specialize incare for certain complex patient types through advanced trainingand education. As an experienced therapist this position provideseducation to patients patients family members and / or caregivers andcollaborates with other interprofessional team members as neededfor problem resolution and / or achievement of identified patientspecific goals.

As a member of the interprofessional care team thePT II position provides patient care directly and deliversexceptional patient care with adherence to safety standards andguidelines to ensure optimal patient safety customer service andoptimum patient outcomes.

This position serves as preceptor mentorand resource to less experienced staff including other Therapistsand technicians and may act as a clinical resource to students andclinical fellows.

The PT II position assists with collection ofquality metrics and analytics with development of best practicesand process improvement initiatives asdesignated.

PEOPLE ESSENTIALFUNCTIONS

Contributes toteamwork consistently offering assistance and responding positivelyto requests for assistance. Collaborates to foster healthyrelationships in the work environment.

Offers guidance tointerprofessional health care team when appropriate and appliesfeedback to improve patientoutcomes.

Uses therapeuticcommunication which includes active listening and teaching toestablish a relationship with patients families andinterprofessional health care team to collaborate on the plan ofcare.

Partners with members of the interprofessional care team byactively communicating and reporting pertinent patient careinformation and data in a comprehensivemanner.

Teaches andprovides guidance to nursing and support staff. Orients new teammembers and assists with precepting students in clinical rotationsand clinical fellows.

Uses peertopeer accountability towardsimprovement of department score for employeeengagement.

SERVICEESSENTIAL FUNCTIONS

Performscomplete and comprehensive evaluation of patients across a broadrange of diagnosis and acuities to identify patients level offunction and to determine the treatment plan.

Develops the plan ofcare based on evaluation findings and implements the physicaltherapy treatment plan. Provides service to complex patientpopulations.

Practices in a manner that is congruent with culturaldiversity and inclusionprinciples.

Provideseducation to patients patients family members and / or caregiversbased on learning needs. Consistently evaluates the patientscomprehension and adapts teaching methods accordingly.

Serves asresource to clinical fellows and nursing in teaching bestpractices.

Collaboratesand communicates with the healthcare team modifications to patientstreatment plan progress and discharge planning for achievement ofidentified patient specific goals.

Advocates on patient / familysbehalf to identify and resolve clinical and ethical concernsutilizing appropriateresources.

Balances andadjusts as needed the caseload for the assigned team members toresult in optimum patient care and outcome. Initiates improvementsfor department score on patient satisfaction through peertopeeraccountability to servicestandards.

QUALITY / SAFETYESSENTIAL FUNCTIONS

  • Assessespatients regularly monitors progress towards treatment goalsmodifies goals and plan or program as indicated. Recognizescomplications / limitations associated with certain procedures andproactively addresses such for optimumoutcomes.
  • Provides selectoversight of physical therapyrelated activities provided byinterprofessional health care team and if necessary adjusts theplan to improve quality of outcome and maintain the safety of thepatient.
  • Proactivelyidentifies opportunities for corrective action and assists inprocess improvement by providing recommendations and contributes tomeeting department and hospital targets for quality and safety.

Identifies areas of practice improvements with researchbasedevidence and assists in development of treatment protocols and bestpractice for certain population or practice area to achieve patientsafety outcomes supported by accuratedocumentation.

FINANCEESSENTIALFUNCTIONS

Maintains accurateand complete documentation of patient education and proceduresperformed in a timely manner in the patient record.

Enters accuratecharges for services provided asapplicable.

Uses resourcesefficiently to maximize utilization of time and equipment.Selfmotivated to independently manage time effectively minimizingincidental overtime and prioritize daily tasks assisting coworkersas needed.

Meets labor productivity standards by effectivelymanaging established patient loadassignments

GROWTH / INNOVATIONESSENTIAL FUNCTIONS

Initiatesinnovative solutions through evidencebased practice / performanceimprovement projects. Actively participates in and may leaddepartment projects and / or hospital committees.

Participates inHMrelated and approved hospital / community teaching opportunitiescommunity services and / orfunctions.

Identifies andpresents areas for innovation efficiency and improvement in patientcare or unit operations using evidencebased practice literature.

Assists higher level PTs with program development and / or clinicalcompetencies.

Seeksopportunities to identify selfdevelopment needs and expand learningbeyond baseline competencies with a focus on continual developmentas a Physical Therapist taking appropriate action.

Ensures owncareer discussions occur with appropriate management. Completes andupdates the My Development Plan on an ongoingbasis

EDUCATION

Graduateof education program approved by the credentialing body for therequired credential(s) indicated below in the CertificationsLicenses and Registrations section.

WORKEXPERIENCE

Two years ofexperience as a Physical Therapist; or completion of HM orcomparable Physical Therapy Residencyprogram

LICENSES ANDCERTIFICATIONS REQUIRED

  • BLSBasic Life Support (AHA) and
  • PT PhysicalTherapist State Licensure with the Texas Board of Examiners.Compact license acceptable according to current ECPTOTErequirements within 90 days or
  • PTTemp PhysicalTherapist Temporary State Licensure with the Texas Board ofExaminers. Compact license acceptable according to current ECPTOTErequirements within 90 days

KNOWLEDGE SKILLS ANDABILITIES
