Job Summary :
We are seeking a highly motivated and experienced Director to oversee the day-to-day operations of our care management programs.
This position will be focused on supporting the Enhanced Care Management (ECM) program, which is a program under the California Advancing and Innovating Medi-Cal (CalAIM) initiative.
The ideal candidate will have a strong focus on accountability and building positive relationships with clients to ensure the highest level of quality care.
As the Director of ECM, you will be responsible for managing the daily operations of the department, providing leadership and guidance to the staff, and ensuring that all policies and procedures are followed.
You will work closely with other departments to provide seamless care to clients and ensure that all services are delivered in an efficient and effective manner.
Essential Functions :
- Develop and implement policies and procedures to ensure quality care and compliance with regulatory guidelines.
- Establish and maintain positive relationships with clients to ensure their needs are being met and achieve high satisfaction.
- Provide leadership and guidance to the care management staff, ensuring they are meeting performance standards and goals.
- Oversee the coordination of care for clients, including assessments, treatment plans, and follow-up care.
- Ensure that all documentation is completed accurately and in a timely manner.
- Collaborate with other departments to ensure that clients receive comprehensive care and services.
- Monitor and evaluate the effectiveness of the ECM program and services provided.
- Analyze data and provide reports to senior management on the performance of the programs.
- Develop and implement strategies to improve client outcomes and satisfaction.
- Leads the services provided to the eligible and enrolled members and ensuring ongoing compliance with the state’s regulations & company’s policies and procedures.
- Growth mentality : Able to lead existing ECM teams and grow the teams regionally and possesses expansion vision to help the company grow vertically with existing clients and horizontally with new clients.
- Develop KPIs that align the teams with the company’s goals and ensure that the teams meet the monthly KPIs.
- Inspire team members and expand the program in the assigned region.
- Fosters positive employee relations through development of an engaging, diverse, team-oriented culture.
- Supports the outreach process to achieve or exceed established goals.
- Strategically manages the operations’ budget to meet or exceed established goals.
Required Skills / Abilities :
- Able to travel approximately 2 weeks per quarter for team meetings.
- Able to steer the overall direction of the team but also in firm command of the details.
- Proven ability to execute and deliver on timelines.

- Knowledge of federal, state, and local regulatory requirements as they relate to case management & social services and operate within these guidelines.
- Proven success managing budget including labor, expenses & capital expenditures.
- Flexibility to work weekends & evenings as the business requires.
Education and Experience :
- Minimum 5 years of comparable case management experience in a healthcare setting.
- Expert in healthcare rules and regulations.
- CalAIM experience highly preferred.
