Position Summary : Licensed Social Worker. The successful performance of this job requires an individual to care about the elderly and desire to assist them and their families yet to be able to maintain a professional approach.
The job also requires high standards in complying with state and federal regulations.
Social Service Tasks
- Adheres to the National Association of Social Workers Code of Ethics.
- Completes a social history, social service care-plan, minimum data set, and discharge plan for new residents on designated floors.
- Completes admission paperwork on designated floors in consultation with residents and their families.
- Maintains necessary documentation in resident’s medical records as required to meet Federal and State regulations and Cassia Home Standards.
- Participates and facilitates care conferences involving the planning and implementation of the interdisciplinary care-plan for residents on designated floors.
- Assists residents and families with personal clothing and care needs, roommate concerns, community resources and financial concerns.
- Completes referrals for resident mental health services and coordinates consultative services of mental health providers.
- Completes new employee orientation.
- Assist Director of Social Services in coordinating other social service activities, such as in-services, family newsletter, family council, etc.
- Assist residents and family with discharge planning : arranging discharge conference, community referrals, etc. on designated floors.
- Completes recapitulation form on discharged and deceased residents.
- Coordinates room transfers and obtains written notice for room changes per policy.
- Complete annual Social Service assessment.
- Assist residents and families as needed using one to one visits, groups, etc.
- Attend daily Admission meeting, weekly staff meetings, and other meetings as required.
Safety / Compliance
- Accepts and follows through with all policies and procedures.
- Identifies and corrects hazards and unsafe acts and reports all quality issues.
- Must be aware of and adhere to all HIPAA regulations.
- Adheres to Cassia policies on infection control, safety and resident rights.
- Completes all Relias and other training required.
- Identifies compliance concerns, reports to supervisor or the Chief Clinical and Compliance Officer / Compliance hotline.
- Participates in facility QAPI, Quality Assurance Performance Improvement program and activities.
Additional8. Eligible to work at a site that receives federal funding.9. Possess skills in problem definition / resolution.
10. Has regular and reliable attendance.11. Participates in required facility in-services and staff training sessions.12.
Participates in facility committees as directed.13. *Performs other duties and responsibilities as directed by supervisor.
Knowledge / Skills / Qualifications :
- College Degree or 1 year experience working in a Social Service related field preferred
- Oral and written communication skills
- Must be eligible to work in the Federal Health Program
Mental Requirements :
- Reasoning : able to solve a wide range of intellectual and practical problems
- Mathematical : able to perform addition, subtraction, multiplication, and division of whole numbers
- English Language : able to compose original correspondence; follow technical manuals; evaluate technical data; counsel or advise people
