Job Title : Certified Occupational Therapy Assistant

Shift : Monday to Friday, Days

Hours per Shift : Guaranteed 32 hours

Experience : Minimum years of experience required. New graduates welcome.

License : Valid state license required.

Certifications : Certified Occupational Therapy Assistant certification required.

Must-Have :

SNF experience preferred.

COVID test required 72 hours prior to start date with results issued 48 hours prior to start date.

Description :

We are seeking a Certified Occupational Therapy Assistant.

Candidates may take on a therapy coordinator role.

It is essential that the prescreen is filled out and attached upon submitting the candidate.

Resume and license verification also need to be attached for consideration.

If these items are not completed and attached, the candidate will not be considered.

Candidates must undergo a credentialing checklist prior to submission, including :

State license verification.

Resume submission.

Three references.

County criminal background check (7 years for all counties lived and worked).

Federal criminal background check (7 years or more).

Multi-state criminal search (7 years or more).

Nationwide sex offender search (7 years or more).

HHS / OIG search.

GSA search.

Social Security number trace.

1 step PPD or Quantiferon or proof of positive PPD and chest X-ray.

SAM search.

Oklahoma state background with fingerprints required.
