- Monitor patient vital signs and report any changes in condition to the healthcare team.- Educate patients and families on health conditions, post-discharge care, and ongoing support needs.
- Maintain accurate and complete patient records in compliance with healthcare regulations.- Ensure a safe and therapeutic environment for patients, staff, and visitors.
