Overview :

We are currently hiring a compassionate Licensed Practical Nurse (LPN) for a full-time, night shift role providing one-on-one care in the patient's home.

This position offers 8pm-8am shifts, Monday through Friday. If you thrive in delivering personalized, patient-focused care, this is the ideal role for you.

Patient Care Needs : G-Tube care

G-Tube care

Diabetes management

Care for a quadriplegic patient

Dependent for daily living assistance

Must be comfortable working in a home with a dog

Qualifications : Current LPN license

Current LPN license

Responsibilities :

Perform selected nursing procedures within the scope of LPN practice

Provide patient and family education according to plan as outlined by the RN

Monitor patients vital signs and make specific measurements and observations

Monitor and record intake and output, and report significant changes immediately

Observe, assess and record patients physical and mental condition and report any changes

Administer medications as instructed and ordered by the physician and instructed by the RN

Attend and participate in staff meetings and in-service education programs, as assigned

Qualifications :

Active LPN license in good standing

Previous home health, private duty, skilled nursing or hospital experience preferred

Ability to build rapport with patients and family members

Strong problem solving and critical thinking skills

Reliable transportation or valid drivers license with current automobile insurance
