Job Title : Adult Health and Day Care Center Nurse

Job Description : Position Summary : As a Nurse at the Ventura Adult Health and Day Care Center, you will play a vital role in providing compassionate care and support to our clients.

You will work closely with a multidisciplinary team to ensure the physical and emotional well-being of our adult participants.

Your primary responsibilities will include administering medications, assessing health conditions, and implementing care plans tailored to individual needs.

Key Responsibilities : 1. Medication Administration : - Administer prescribed medications accurately and safely according to established protocols.

Monitor clients for any adverse reactions or side effects and promptly report any concerns to the appropriate medical personnel.

2. Health Assessment : - Conduct regular health assessments to monitor clients' physical and mental well-being. - Document and maintain accurate records of clients' health status, including vital signs, medical history, and any changes in condition.

3. Care Planning : - Collaborate with the interdisciplinary team to develop and implement individualized care plans for each participant.

Identify specific needs and preferences of clients and incorporate them into the care plan to enhance their quality of life.

4. Health Education : - Provide health education and guidance to clients and their families on topics such as medication management, disease prevention, and healthy lifestyle choices.

• Offer support and resources to empower clients to actively participate in their own care. 5. Emergency Response : - Respond promptly and effectively to medical emergencies or incidents within the facility.
• Follow established emergency procedures to ensure the safety and well-being of clients and staff members. 6. Collaboration and Communication : - Communicate effectively with clients, families, caregivers, and team members to coordinate care and address concerns.
• Participate in team meetings and case conferences to discuss clients' progress, goals, and any necessary adjustments to the care plan.

Qualifications : - Valid nursing license (RN or LPN) in the state of California. - Previous experience working with older adults or individuals with disabilities preferred.

• Strong clinical skills and knowledge of nursing principles and practices. - Excellent communication and interpersonal skills.
• Ability to work collaboratively in a multidisciplinary team environment. - Compassionate and patient-centered approach to care.
• CPR and First Aid certification required. Working Conditions : - The role involves working in a dynamic and fast-paced environment with a diverse population of clients.
• May require standing, walking, bending, and lifting. - Possible exposure to infectious diseases and challenging behaviors.
