Nurse Case Manager (RN) Responsibilities :
- As a nurse case manager (RN), you will be responsible for the managing resident cases.
- You will evaluate resident's medical history, needs, and support network.
- You will chart and maintain documentation for residents.
- You will design resident treatment plans.
- You will develop and maintain a positive working rapport with all team members.
Requirements :
Nurse Case Manager (RN) Qualifications :
- Registered Nurse in the State of Illinois.
- No history of abuse or neglect
- Current CPR certification.
- At least three years experience in long term care.
- A positive communication style
