Overview
Purpose Statement / Position Summary : The Program Manager oversees the patient and family education program, with responsibility for planning, implementation, and evaluation of the patient / family education courses for patients across the care continuum. Leads regular needs assessments to understand changes in patient populations and associated family learning needs and quality / safety trends for home-based care. Serves as a consultant to inpatient, ambulatory, and community settings to ensure continuity of care and programmatic offerings that minimize harm in the home setting and optimize patient outcomes and the patient / family experience. Leads continuous quality improvement initiatives to ensure that educational content is accessible, relevant, and evidence-based and uses a multimodal teaching approach to meet learning needs of a diverse population of patients and families. Tracks, analyzes, and reports program metrics to demonstrate impact and program sustainability.
