Status : Non-exempt (Hourly), Full-Time, Non-Management

Schedule : Monday-Friday 8 : 00am-4 : 30pm

Setting : Hyrbid

Location : The Nest 222 S. Hill St. 7th Fl., Los Angeles, CA 90012

SUMMARY

As part of a multi-disciplinary team, the Intensive Case Management Services (ICMS) Clinical Case Manager will travel to multiple locations in the greater Los Angeles area to meet and work with clients to attain and sustain permanent housing in partnership with the County Department of Health Services (DHS).

The ICMS Clinical Case Manager will work with clients to provide intensive field-based case management and clinical services while employing a trauma-informed care and harm reduction approach to a diverse group of clients with complex trauma facing medical, mental health and substance use barriers.

Client remain in the program for as long as they require subsidized housing. For many individuals this may mean their entire lifetime.

ESSENTIAL DUTIES & RESPONSIBILITIES

1. Work with a diverse and marginalized client caseload of 20 clients experiencing many barriers including homelessness, complex trauma, medical, mental health and substance use

1. Assess mental health service needs of clients and provide therapeutic services or link to appropriate entity

2. Provide crisis intervention and safety assessments to clients on caseload

3. Link clients with appropriate medical, mental health, substance abuse, and vocational resources

4. Assist clients in obtaining all benefits to which they are entitled

5. Opportunities to create and facilitate support groups focusing on housing stability, community integration and improvement of mental health

6. Ensure services to clients are reflective of the Whatever It Takes model, which emphasizes customer choice, harm reduction, trauma-informed care and psycho-social rehabilitation

7. Ensure that each client on caseload has an up-to-date and comprehensive biopsychosocial assessment, and that this assessment is used in collaboration with the client to create individualized goal plans designed to improve quality of life and improved health outcomes

8. Collaborate with each client to develop creative goal plans aimed at improving overall well-being and housing stability;

review and update quarterly and upon completion of goals

9. Develop and maintain a complete, accurate, and current client file with all required documents and data in agency records and electronic databases

10. Document all client contacts in PIP format (Purpose, Intervention, Plan) and in accordance with Housing Department productivity expectations

11. Maintain confidentiality of client files per HIPAA and all applicable guidelines

12. Transport clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community reintegration

13. Assist in the screening, assessment, and enrollment of clients, including orientation to program policies, resources and goals

14. Maintain a current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to clients

15. Work cooperatively and cohesively with other clients of the staff team, including participation in weekly staff meetings and staff trainings

16. Participate in weekly individual and group clinical supervision

QUALIFICATIONS

1. MSW, MFT or LPC who is BBS eligible (supervision hours are available for this position)

2. Registered with the board of behavioral sciences or ability to do so within the first 2 months of employment

3. Demonstrated knowledge of clinical case management

4. At least one year experience working with clients experiencing homelessness and with mental illness, chronic health issues, and substance use disorders.

5. Knowledge of community resources

6. Able to work in a high-tolerance, harm reduction model with individuals who have multiple barriers

7. Strong clinical, team-building, charting, and non-violent crisis intervention skills

8. Computer literate; able to effectively use computerized database for client file management, with basic skills in Microsoft Word, Outlook, and Excel programs

9. Detail oriented with strong time management, organizational, written, verbal, critical thinking and interpersonal skills

10. Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record

11. Able to transport clients in your vehicle

12. Minimum 30 WPM typing speed preferred

WORK ENVIRONMENT

1. Combination of field and office environment

2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.

3. On occasion walk or drive to different local sites

4. Will necessitate working in busy and loud environments

5. Will be exposed to elements like cold, heat, dust, noise and odor

6. May need to bend, stoop, twist, and sit throughout the day
