The Licensed Practical Nurse - In-Home Care is responsible for providing direct nursing care to assigned patients under the supervision of a registered nurse (RN) or other provider.
This role involves monitoring vital signs, performing dressing changes, collecting specimens, and communicating with physicians regarding patient updates.
- Provide direct nursing care to patients under the supervision of a registered nurse, midlevel provider, or physician, following the nursing plan of care.
- Report patient response to care to registered nurse or other provider for evaluation and modification of the plan of care.
- Ensure complete and accurate documentation of care per regulatory requirements and department policy.
- Perform complex dressing changes, venipunctures, and foley catheter changes as ordered.
- Communicate with MDs regarding patient updates or changes in condition.
- Educate patients on enteral feedings and care of gastrostomy or jejunostomy tubes.
