Trilogy Home Healthcare is an exciting place to be right now! We are experiencing tremendous growth, cover over 42 counties and have 11 branch offices with a census of anywhere between 300-800 plus patients.

We are looking for Social Workers to provide the highest quality of care for our home health patients in a professional and compassionate manner.
The ideal person will be specifically trained, competent, and perform assigned functions of care to the patient under the direction and supervision of the Director of Clinical Services and Clinical Team Manager.
They must also be well connected knowledgeable about community, state, and federal resources for our patients. This rewarding job allows you to be a part of patients' lives and make a huge difference for them every day!

Qualifications :
- Master's degree from a school of social work accredited by the Council on Social Work Education (MSW) or Clinical Social Worker license (LCSW)
- Three to five (3-5) years' experience preferred, must be well connected in the community and an understanding of community offerings for patients
- One (1) year experience in home care preferred.
- Ability to exercise initiative and independent judgment.
- Ability to work with individuals to enlist cooperation of many people to perform / achieve a common goal.
