General Purpose :
Responsible for providing personal care services to hospice patients, as well as support in incidental / instrumental activities of daily living under the supervision of a Registered Nurse (RN) and in accordance with the policies and procedures of the Facility and applicable state and federal law or regulation.
Essential Functions :
- Provides services as stated in each patient’s Hospice Aide care plan.
- Assists patients in safely performing incidental / instrumental activities of daily living.
- Assigned duties may include but are not limited to :
- Providing personal care : bathing, oral hygiene, hair care and skin care.
- Assisting in dressing and undressing patients.
- Assisting in feeding the patient.
- Offering and assisting with toileting and elimination.
- Utilizes infection prevention and control measures such as universal precautions, hand washing, and personal protective equipment.
- Recognizes, reports, and records / documents changes in patient condition and safety to patient’s assigned RN.
- Organizes self to carry out visits / shifts and tasks.
- Documents observations, activities and services provided in an accurate, complete and timely manner..
- Completes other assignments as requested and assigned.
- May have access to personal health information ( PHI ) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization / department.
The Interim HealthCare Home Health Aide MAY NOT :
Perform any skilled nursing procedure, which is prohibited by the State Nurse Practice Act.
Minimum Education & Experience Requirements :
- Eighteen (18) years of age or older.
- Graduate of a training and competency program or competency program as required by State regulations.
- Ability to perform and prioritize multiple tasks or functions.
- Meets applicable health requirements to provide patient care.
- Able to effectively communicate to clinical and non-clinical individuals.
- Facility and / or Hospice experience a plus.
- Reliable transportation necessary.
