Job Summary and Qualifications

Provides coordinated skilled nursing care to patients ofall age groups, in the home. Demonstrates accountability andresponsibility in collaborating with the interdisciplinary team toestablish and achieve patient goals and maintain high qualitypatient care.

Performs in accordance with physician's orders andunder the supervision of the ClinicalManager.

What you will do in thisrole :

• Assesses homecare patients identifying physical, psychosocial and environmentalneeds as evidenced by documentation, clinical records, caseconferences, team reports, call-in logs and on-siteevaluations.
• Completes OASIS, assessment andvisit paperwork according to agency policy. Assures clinical notesaccurately indicate continuing communication and coordination ofservices with the physician, other interdisciplinary team membersand patient / family / caregiver.
• Communicatessignificant findings, problems and changes to Clinical Manager andphysician, and documents all findings, communications, andappropriate interventions.
• Supervises andprovides clinical direction to home health aides and LPNs / LVNs toensure quality and continuity of servicesprovided.
• Responsible for participating inon-call rotation and emergency call according to agencypolicy.

• Basic CardiacLife Support must be obtained within 30 days of employment startdate
