Job Summary :

The Wound Ostomy Continence Nurse (WOCN) serves as a clinical expert and consultant, providing care to patients with ostomies, wounds, fistulae, incontinence, and skin breakdown.

The role involves utilizing the nursing process to care for patients across the pediatric, adolescent, adult, and geriatric populations in an acute care setting.

The WOCN collaborates with the healthcare team, patients, and families, exercising independent clinical judgment and providing recommendations for optimal patient outcomes.

Key Responsibilities :

• Assess and plan individualized care for patients with complex wound, ostomy, and continence issues.
• Provide direct care and consultation under physician orders, implementing appropriate wound care treatments and therapies.
• Educate clinical staff on wound care protocols and ongoing patient needs.
• Monitor, document, and report patient progress, collaborating with the Clinical Manager on care plans and performance improvement initiatives.
• Manage negative pressure therapy units and coordinate patient care supplies and orders.
• Promote standardization of care practices and identify cost containment opportunities.

Additional Skills :

• Strong critical thinking, clinical judgment, and decision-making skills.
• Ability to effectively communicate and collaborate with physicians, clinical staff, patients, and families.
• Experience in negotiating and motivating others to achieve optimal care outcomes.

Additional Information :

• The position requires a 100-mile radius rule for eligibility.
• Must have the ability to work efficiently in a team setting while addressing patient needs in a timely manner.
