Key Responsibilities

Participates in care planning with patients and healthcare teams to improve quality of life

Assesses patient knowledge and social determinants of health affecting treatment adherence

Provides education and counseling on chronic kidney disease and assists with referrals to community resources

Required Qualifications

Master's degree in social work

Active and unrestricted state-specific licensure as a Licensed Clinical Social Worker

Experience in a renal setting working with ESRD and CKD patients

Basic computer skills and understanding

Strong understanding of insurance, Medicare, and Medicaid processes
