Job Summary

Provide direct primary health care to patients, in collaboration with consulting physician and the multidisciplinary health team in accordance with privileges approved by the credentials committee, to include identification, management, and / or referral of health problems.

Responsibilities

The APN works in the Student Wellness Center and manages straightforward to moderately complex medical problems and encourages health promotion and disease prevention.

With a focus on providing holistic, integrated care, the APN collaborates with other health professionals to provide high quality care for students at the University of Chicago.

Conducts comprehensive physical examinations, gynecological or episodic health assessments and develop patient care plans within clinical nursing practice and medical care protocols.

Takes patient histories and conducts physical examinations; screens patients to determine the need for hospital admission, making referrals to other disciplines as indicated, writes progress notes;

prepares patient workups and summaries noting pertinent positive and negative findings; obtains psychosocial history relevant to patient care, and assumes responsibility for developing therapeutic relationships with patients.

Orders and schedules laboratory studies and diagnostic procedures; explain necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.

Evaluates outcomes of care delivered to patients and initiates changes accordingly.

Participates in development of collaborative practice arrangements.

Assumes accountability for patient care in making decisions, prescribing appropriate medications as set forth in the delegation of prescriptive authority, prescribing appropriate treatments, delegating tasks, and maintaining a comfortable environment for the patient.

Collaborates with and acts as liaison among the nurse, patient, family, physician, and community resources for coordination of patient care.

Selects appropriate communications approach to achieve desired outcomes of patient care.

Actively pursues required knowledge and skills for professional development.

Identifies and participates in quality assurance initiatives.

May participate in health education in the community.

May participate in basic science and clinical research projects conducted by supervising physician.

May prepare statistical reports and analyses setting forth progress, adverse trends and appropriate recommendations or conclusions.

May participate in teaching students and residents in techniques and procedures which are unique to specific clinical setting.

Acts as a resource for clinical skills to staff inside or outside the unit. Practice specific and invasive procedure privileges are detailed in each APNs Delineation of Privileges.

Interprets assessments, treatments, outcomes, work and disability status, and screenings in a thorough manner. Maintains clear and concise records to meet the requirements of the department and any applicable standards or laws.

Analyzes test data or images to inform diagnosis or treatment.

Participates in counseling patients about prescribed therapeutic regimens, family planning, emotional problems of daily living, and health maintenance.

Interprets patient's condition quickly and concisely and determine the best course of treatment. This can include ordering appropriate laboratory or imaging studies, discussing patient with the Emergency Physician and / or referring patient to consultants.

Performs other related work as needed.

Minimum Qualifications

Education :

Minimum requirements include a college or university degree from an accredited school / college.

Work Experience :

Minimum requirements include knowledge and skills developed through a combination of education and training in a related job discipline.

Certifications :

Current and / or eligible for State of Illinois licensure / registration required. Eligibility to be privileged and credentialed through the Medical Staff Office (MSO). - State of Illinois

Preferred Qualifications

Education :

Completion of an accredited Nurse Practitioner program.

Experience :

Experience in primary care, urgent care, family medicine, gynecology or equivalent ambulatory setting preferred.

Licenses and Certifications :

Eligibility to be privileged and credentialed through the Medical Staff Office (MSO).

Current certification as Family or Adult Nurse Practitioner by the American Nurses Credentialing Center, or equivalent certifying body.

Current Registered Nurse (RN) license and Certified Nurse Practitioner with prescription privileges license in Illinois.

Current Healthcare Provider CPR certification.

Technical Skills or Knowledge :

Demonstrated clinical competency, effective leadership and teaching skills.

Ability to conduct physical examinations and health assessments.

Ability to interview and obtain medical information.

Willingness to enter into written collaborative agreement with a collaborating physician upon hire.

Once licensed and as needed, willingness to apply for a mid-level practitioner controlled substance license and DEA number.

Willingness to be privileged and credentialed through UCM medical staff office prior to start date.

Proficiency in the use of computers.

Working Conditions

Ability to sit or stand for 4 hours or more.

Ability to use computers extensively for 4 hours or more.

Work may involve assisting patients in transferring to and from chairs, wheelchairs, exam tables and gurneys. Current lifting procedures must be followed.

Duties performed require potential exposure to blood-borne pathogens.

Occupational risk for exposure to infectious material and communicable diseases.

Scheduled work hours may change depending on clinic needs. Full-time clinical providers in the Student Wellness Center are expected to work a 40-hour work week, but as an exempt position, this number may fluctuate depending on the needs of the operation.

Administrative time will be done from the office. The regular work schedule is Monday-Friday and approximately eight Saturdays per year.

The actual number of Saturday work days may increase or decrease depending on the needs of the operation. Infrequent on-call and evening rotations may be requested.

Work may be performed in varied locations inside / outside of the clinical setting including other campus or offsite locations.
