About The Position
Job Title : Rehabilitation Therapist Job Specialty : Occupational Therapist Job Duration : 13 Weeks Shift : Monday to Friday Guaranteed Hours : 32 Hours per Week Experience : New Graduates Welcome, Skilled Nursing Facility Experience Preferred License : Must Have Valid State License Certifications : Valid Occupational Therapy Credentials Must-Have : Prescreen Form Completed and Attached- Resume and License Verification Attached- COVID Test Required 72 Hours Prior to Start Date Job Description : Conduct comprehensive assessments to evaluate patient needs.
- Develop and implement personalized treatment plans.- Assist patients in improving their daily living and work skills.- Monitor and document patient progress and modify treatments as necessary.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.- Maintain a clean and safe working environment for patients.
- Follow required credentialing checklist for background checks and health screenings, including but not limited to : - Social Security Number Trace - Criminal Background Checks (County, Federal, Multi-State, Nationwide Sex Offender Search) - Health and Human Services / Office of Inspector General Search - General Services Administration Search - System for Award Management Search - Tuberculosis Testing or Proof of Positive Test and Chest X-Ray- Provide compassionate and quality care to all patients.
