Impactful Work : Help individuals with physical disabilities, mental illness, and developmental disabilities gain independence and thrive in their communities.

Dynamic & Flexible : This role involves traveling within the community, providing direct, hands-on support and assisting the Designated Coordinator with key tasks.

Crisis Prevention & Support : Play an essential role in crisis prevention, ensuring individuals receive the care and support they need, especially during emergencies.

Team Collaboration : Work closely with the Designated Coordinator and care team to ensure smooth service delivery and compliance with regulations.

Please Note : The duties and responsibilities of this role may change in the near future due to program adjustments. While we are unable to predict exactly how the role will evolve, we are committed to ensuring transparency and support during any transitions.

Candidates should be flexible and adaptable to potential changes in job responsibilities. Key Responsibilities (Current) : Provide direct care and support to individuals living independently, helping them acquire essential adaptive skills.

Assist individuals in developing personalized 24-Hour Emergency Back-Up Plans. Support the Designated Coordinator with clerical tasks, ensuring efficient service delivery.

Document progress, report incidents, and maintain compliance with 245D regulatory guidelines. Collaborate with a wide range of professionals, including waivered service providers, mental health teams, and social workers.

Participate in an on-call rotation for 24-Hour Emergency Assistance services. Comprehensive Benefits Package : 401(k) with company match up to 3.

5% (eligible after 1 year). Health, dental, and vision insurance with employer-paid contributions (eligible after 60 days).

Earned Sick and Safe Time (ESST) : 1 hour for every 30 hours worked. 7 paid holidays and 127.92 hours of Paid Time Off (PTO) .

Strong organizational and time-management skills. Valid driver’s license and access to a reliable, insured vehicle (mileage reimbursement provided) Must pass criminal / registry background checks in accordance with Minnesota DHS guidelines.

Hold at minimum a 2-year degree in Human Services or a related field, AND at minimum 2 years of direct experience working with individuals with Intellectual / Developmental Disabilities or related conditions Working Conditions : The Unit Based Services Program Assistant works in various community settings, frequently traveling to clients' homes.
