ChristianaCare is currently seeking a full-time Registered Nurse to join the Family Medicine Center at Wilmington Primary Care located at 501 West 14th Street in Wilmington, DE .
You will be joining a team of physicians, advanced practice clinicians, Registered Nurses, medical assistants, and office assistants to support this very busy residency practice.
Work Schedule :
- Monday through Friday, 8 : 00am to 4 : 30pm and 8 : 30am-5 : 00pm rotation.
- No weekends or holidays.

- Performs Annual Wellness Visits, Advance Care Planning and Transitional Care Management calls and disease management.
- Triages walk-in patients and ensures appropriate disposition
- Telephone triage
- Answers and responds to telephone messages under the direction of the provider
- Assists providers in examinations or procedures
- Functions as a liaison between patients, physicians and staff to provide accurate communication
- Assures adequate coverage within unit when needed
- Supports the work of LPN and / or MA
- Assists with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN)
- Assists with orientation of new personnel to ensure that they are properly trained to support patient and provider.
- Performs chart audits or has designee assist with chart audits
- Case Management and interdisciplinary collaboration
- Chronic disease management under provider direction
- Performs nurse appointments for patient education and medication / vaccine administration
- In office medication administration and wound care
- Full spectrum family medicine practice - meaning prenatal care as well
- Help to support Substance Use Disorder Programs in Family Medicine office and Mobile Van. Includes coordination of care and patient outreach.
Education Requirements :
- A graduate of a state approved or National League of Nursing accredited School of Nursing.
- Two years’ experience as a RN, physician office experience preferred.
Special Requirements :
- Current RN licensure in the State of Delaware
- BLS certification required
