What You'll Do

Position Summary

The Guia Manager is responsible for a panel of patients and, in collaboration with other members of a multidisciplinary primary care team, helps patients meet their preventive, chronic, and acute care needs.

The Guia Manager will engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt lifelong healthy behaviors.

In addition to performing their regular Guia functions, the Guia Manager serves as a role model and leader among peer Guias.

This individual's primary responsibilities center around establishing trusting, supportive, collaborative relationships with co-workers, patients, and their families.

Our Guias build relationships with patients in a clinical setting and in the community by working alongside medical providers, nurses, other Guias, medical assistants, and behavioral health specialists in a collaborative and empathetic team to improve patient outcomes.

Guia Managers play a key role by overseeing a team of 8 12 Guias that carry out these daily functions.

Responsibilities

Essential responsibilities consist of but not all inclusive :

  • Lead a team of 8 12 Guias and support them through any problems affecting the care of patients, conflict resolution, and general coaching.
  • Deliver Suvida specific training for new Guias; co-create new training modules with Guia Director if needed and provide program feedback on training, protocols, and other policies.
  • Ensure Guias meet key performance indicators and that they remain up to date with training and certifications.
  • Track and manage Guia caseloads, data reports, patient enrollment, progress towards goals, program expenses, and other program features.
  • Recruit, interview, and supervise Guias.
  • Build and maintain effective relationships with other care team members that result in timely and appropriate patient follow-up.
  • Work with Community Liaison to develop relationships with various organizations (e.g., faith-based groups, hospitals)
  • Occasionally supervise home and virtual visits conducted by other Guias to ensure program fidelity
  • Meet with other Guia managers to exchange best practices and attend any mandatory staff meetings / trainings · Assist Guias with any social work needs (e.

g., assisting with a patients' medico-legal issues)

  • If needed, actively engage a panel of assigned patients within Suvida Healthcare during the clinic's ramp-up phase, and function as a back-up thereafter.
  • Other duties as assigned by the Guia Director

What You'll Bring

Knowledge, Skills, and Abilities

  • Experience in a customer service environment.
  • Clinical experience, such as being a LCSW.
  • Management experience, such as leading teams
  • Able to articulate Suvida Healthcare's mission in relation to patient satisfaction and patient outcomes.
  • Excellent interpersonal communication skills with exceptional listening abilities
  • Compassionate, kind, and open-minded
  • Teamwork experience
  • Proficient with computers, mobile devices, medical devices, and Microsoft Office Suite
  • Written communication skills
  • Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
  • Ability to work independently and exercise sound judgment in interactions with providers, payors, patients, and their families.
  • Effective oral and written communication skills
  • Able to care for patients in-home, in-clinic, and remotely.
  • Bilingual / Bicultural (English and Spanish) Preferred

Education, Experience, Licensure, or Certification Requirements

  • Bachelor's degree in healthcare or related field
  • Master's Preferred.
