About The Position
Job Title : Care Coordinator Social Worker Job Specialty : Care Management Job Duration : 13 weeks Shift : Day, 5x8 Mon-Fri, with weekend and holiday rotation Guaranteed Hours : N / A Experience : Minimum 1 year of experience in healthcare in an acute care setting, related field, skilled care environment, or community or educational internship in care coordination.
A background in medical social work in an acute care setting is preferred. License : Social Work License Certifications : Basic Life Support (BLS) Must-Have : - Excellent written and verbal communication skills - Ability to function well under stress, handle change, and work in a fast-paced environment - Strong assessment, interview, organizational, and problem-solving skills - Knowledge of local, state, and federal regulations and community resources - Ability to collaborate with physicians, care team members, and patients / families effectively Job Description : - Assess transitional care needs and coordinate care across the continuum - Engage with patients and families to ensure care needs are met - Provide psychosocial assessments and advance discharge planning - Relay information about community-based services and offer guidance to patients / families - Serve as a specialist on psychosocial and discharge needs, end-of-life care planning, and resource needs - Partner with RN Case Managers concerning complex social health issues - Implement and document timely discharge planning and resource information - Understand eligibility processes and criteria for resource planning - Respond to referrals for patient assistance from care team members - Participate in interdisciplinary rounds for discharge planning - Facilitate post-acute referrals for care transitions - Provide financial needs assessments for follow-up care assistance - Advocate with patients and families for autonomous health care decisions - Ensure culturally and religiously sensitive service and continuity of care - Participate in the development of protocols and performance improvement to optimize patient outcomes - Complete and document initial psychosocial / functional assessments - Maintain up-to-date and clear documentation of progress notes and patient / family interactions - Track avoidable days and report trends in undesired outcomes - Complete all initial and ongoing professional competency assessments and mandatory education - Serve as a preceptor or mentor for social work students, as appropriate - Perform other duties as assigned - Comply with all organizational policies, standards of work, and code of conduct
