Care Coordinator

In this role, you'll be responsible for providing patient-centered care coordination and assistance in navigating the complex healthcare system.

The ideal candidate for this position will have a deep commitment to patient care, exceptional communication skills, and a passion for making a difference in the lives of others.

You will need to be a detail-oriented, proactive problem solver who can manage multiple tasks in a fast-paced environment.

Key responsibilities of the Care Coordinator role include :

  • Assisting patients in navigating the healthcare system by scheduling appointments, completing medication requests, and managing referrals to specialists
  • Coordinating care with other healthcare providers and community resources to ensure patients receive comprehensive, integrated care
  • Ensuring all patient demographic information is updated and accurate in the EHR System
  • Triaging patients to the most appropriate point of care based on urgency and protocols
  • Developing solutions to customer service issues with the guidance and direction of your immediate supervisor and team members
  • Participating in quality improvement activities to continuously improve the delivery of care and patient outcomes
  • Sending outbound questionnaires to patients via their preference of e-mails / texts working with the Care Management team

Qualifications for this role include :

  • High school diploma or GED
  • Experience with patient-centered care coordination, call center, or just an overall desire to help people
  • Strong communication and interpersonal skills, with the ability to build patient trust
  • Believe you have what it takes to serve, educate, and advocate for our community including the most vulnerable members
