Responsibilities : Position Summary :
Position Summary :
The Clinical Pharmacist's job summary includes assisting the Clinical Coordinator with the following but not limited to : data collection for Pharmacy MERP / PI, disease states clinical program intervention / education.
Educate physicians, nursing, and pharmacy staff on clinical programs. Ensuring patient safety on medication reconciliation process, drug utilization evaluations, and working with the ID physician on antimicrobial stewardship.

Required Qualifications :
Licensed by the California State Board of Pharmacy as a pharmacist with at least a Pharm. D. degree
Continuing education in pharmacy and contact with literature of pertinent nature such as professional scientific journals
5 years' experience in hospital pharmacy practice including pharmacy operations
Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Must demonstrate customer service skills appropriate to the job
Excellent written and verbal communication skills in English
Ability to maintain a work pace appropriate to the workload
Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications :
Familiar with automated dispensing cabinets, unit dose, IV Admixtures, pharmaceutical care, and performance improvement
ASPH accredited residency and or Board Certification of Pharmacy Specialty.
Knowledge of University and medical center organizations, policies, procedures and forms
