Weekend Home Health Registered Nurse - 8k Sign-On Bonus
Job Summary :
Provide home visits for hospice and palliative patients on weekends, working 30 hours paid for 40 hours.
Requirements :
- Graduate of an NLN-approved nursing program.
- Current RN license in Connecticut.
- One year of recent nursing experience in community health / homecare.
Responsibilities :
- Complete physical and psychosocial assessments.
- Establish and implement care plans and goals.
- Refer patients to appropriate therapies.
- Periodically reevaluate patient conditions and progress.
- Coordinate with weekend manager for changes in care plans.
Skills and Competencies :
- Knowledge of professional nursing theory and practice.
- Ability to maintain quality control standards.
- Effective communication and documentation skills.
- Willingness to provide service in any part of Agency geographic area, according to Agency need.
