PURPOSE ANDSCOPE :
Supports FMCNA'smission, vision, core values and customer service philosophy.Adheres to the FMCNA Compliance Program, including following allregulatory and FMS policy requirements.
Ensure provision of qualitypatient care while maintaining cost-effective clinical operationsin accordance with all legal, compliance, and regulatoryrequirements and programs.
Adheres to all requirements of the FMCNACompliance Program, and FMS patient care and administrativepolicies as the clinical leader, has the authority to make dailydecisions to ensure continuity of care and patient and staff safety
PRINCIPAL DUTIES ANDRESPONSIBILITIES :
- Managesthe tactical execution of short- and long-term objectives throughthe coordination of activities with a direct responsibility forresults, including costs, methods, and staffing.
- Provides leadership, coaching and development plans forall direct reports to maintain an engaged and productive workforce;
partnering with Human Resources on employee matters.
Technically proficient in the specific department andknowledge of industry practice and business principles. Works onissues of diverse scope where analysis of situation or datarequires evaluation of a variety of factors, including anunderstanding of current business trends.
Has a larger range withinthe department.
- Responsible for supportingand driving FMS quality standards through meeting all ESRDregulatory requirements and the practice of Quality Assessment andImprovement (QAI), including use of FMS QAI tools.
- Responsible for addressing and acting on adverse eventsand action thresholds.
- Oversees facility'sHome Therapies Program if applicable.
- Accountable for compliance with all applicable federal, state andlocal laws and regulations.
- Ensures all FMSClinical Quality policies and procedures are communicated to andimplemented by the facility staff.
- Maintainsintegrity of medical records and other FMS administrative andoperational records.
- Complies with all datacollections and auditing activities.
- Maintains facility environmental integrity, including safety.
- Experienced leadership required formulti-faceted environment; role primarily focuses on tacticalexecution. Receives assignments in the form of objectives anddetermines how to use resources to meet schedules and goals.
- An individual contributor will havesignificant project / process responsibilities. Program or projectresponsibility generally within the function.
- Follows processes and operational policies in selectingmethods and techniques for obtaining solutions. Interprets andrecommends change to policies and establishes procedures thateffect immediate organization(s).
- Decisionshave an impact on work processes and outcomes. Erroneous decisionsor failure to achieve results will add costs and may impact theshort-term goals of the organization.
- Frequently interacts with subordinate supervisors, customers,and / or functional peer group managers, normally involving mattersbetween functional areas, other company divisions or units, orcustomers.
- Participates and presents atmeetings with internal and external representatives. Often leadinga cooperative effort among members of a project team.
- Interacts with internal departments and externalcustomers; particularly in problem resolution. Acts as an advisorto subordinate(s) to meet schedules and / or resolve technicalproblems.
- Responsible for hiring, coachingand counseling employees, including performance reviews,disciplinary action and terminations.
- Provides technical guidance.
- Performs otherrelated duties as assigned.
PATIENT CARE :
- Coordinates all aspects of patient care with theappropriate staff members, from admission through discharge of thepatient.
- Ensure education of the patientregarding quality measures, transplant options, modality awareness,and access care, including catheter reduction and adherence totreatment regime.
- Acts as a resource for thepatient to address patient concerns and questions.
- Accountable for timely completion of patient careassessments and care plans by organizing meetings of the facility'sInterdisciplinary Team to discuss patient care plans and to resolvepatient problems.
- Directs initiation,maintenance and communication of efficient and timely patientschedules to ensure maximization of the facility stationefficiency.
- Is aware of and develops amechanism or process for knowing the specific situation of eachpatient, including hospitalizations, no- shows, catheter use, andany significant change in patient care status.
- Develops action plans for unexcused and missedtreatments in collaboration with the Director.
- Plans, coordinates and acts as the liaison for patientcare as per the disease management agreement, including initial andongoing validation of member eligibility.
Facilitate timely workupof patients for access management, dialysis services, patienteducation, hospitalizations, and kidney transplantation asappropriate, and ensures coordination of care with themultidisciplinary renal team.
Communicates changes in patientstatus regarding clinical, insurance, travel and transportationissues to the disease management company.
Maintains currentknowledge of disease management software and systems as pertinent.
STAFF :
- Responsible for implementation of FMSstaffing, and medical supply models, to provide quality patientcare, and makes recommendations.
- Participates in the recruitment and interview process, and decisionto hire new personnel.
- Ensures completion ofnew hiring orientation and training including mandatory in-servicesand ICD code training when applicable.
Ensures documentationcompleted for annual in service training, and policy and procedurein service updates.
- Responsible for allpatient care employees receiving appropriate training according toFMS policy, including training to ensure ongoing compliance withall FMS risk management initiatives.
- Provides opportunities for professional growth, and training toensure clinical competence and the ability for licensed staff toassume Team Leader responsibilities
- Responsible for overseeing performance of all licensed personnel,direct patient care staff, reporting indirect patient carepersonnel as assigned, and when necessary, technical staff.
Provides employee education and guidance, and feedback related toperformance.
- Maintains current knowledgeregarding FMCNA benefits, Human Resources policies, procedures, andprocesses, and acts as a resource to facility staff.
- Provides counseling for all clinical staff members atregular intervals offering support and encouraging professionalgrowth.
- Completes timely employeeevaluations and establishes annual goals for staff.
- Provides written documentation of all disciplinaryconferences in accordance with the established personnel policies,and confers with the Director and Human Resources regarding thenature of the disciplinary decisions.
- Participates in Corporate and Business Unit specific employeerecognition and satisfaction programs.
- Creates, maintains, and communicates efficient and timely employeeschedules according to the needs of the facility.
- Creates and implements a Continuous Quality Improvement(CQI) Process Improvement Team that involves staff in problemsolving.
PHYSICIANS :
- Facilitates the application process for physicianprivileges and compliance with FMS Medical Staff By-Laws.
- Responsible for strong Director andphysician relationships and facilitating staff relationships withphysicians.
- Ensures regular and effectivecommunication with all physicians, through regular meetings withDirectors.
- Participates in Governing Body.
- Schedules and coordinates CQI meetings withphysicians.
MAINTENANCE / TECHNICAL :
- Responsible for the integrity and safety ofthe facility water system.
- Must beknowledgeable in the operation of all facility equipment andtechnology.
ADMINISTRATIVE :
- Responsible for maintaining and updating all FMSmanuals.
- Accountable for completion of theAnnual Standing Order Review and ICD coding.
- Checks correspondence whether electronic, paper or voice mail, andresponds as appropriate.
- Directs informationgathering as required supporting billing and collection activities.
- Responsible for efficient utilization ofmedication, laboratory, inventory, supplies and equipment toachieve supply cost goals following all guidelines established inFMS formularies
- Participates in thecompletion and interpretation of the Pl-17 inventory supply useanalysis.
- Reviews and approves facilitypayroll.
- Reviews profit and loss statementswith Director
- Responsible for participatingin all required Network reporting and on-site state or federalsurveys.
- Participates in the completion ofthe FMS Administrative Clinical Review.
