Job Description
We are seeking a motivated and dedicated Per Diem Behavioral Health Case Manager to join our dynamic team. This position offers a flexible and fun environment that supports professional growth and upward movement within the organization.
Responsibilities :
- Conduct comprehensive assessments to identify client needs and strengths
- Develop and implement individualized service plans (ISPs)
- Provide case management services including coordination of care, referrals, and follow-up
- Facilitate group and individual counseling sessions
- Monitor and evaluate client progress, adjusting treatment plans as necessary
- Maintain accurate and timely documentation of services provided
- Collaborate with multidisciplinary team members to ensure holistic care
- Participate in training and professional development opportunities
Qualifications :
- Bachelor’s degree in social work, psychology, or a related field
- Minimum of 2 years of experience in case management or behavioral health services (preferred, but not required)
- Licensed in the state of Florida (Certified Behavioral Health Case Manager) preferred
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in electronic health records (EHR) systems
- Commitment to providing high-quality, client-centered care
