0) Guaranteed Hours : 32 hours / week Experience : One year recent home care experience as a clinical pediatric nurse required;
hospital clinical nurse experience preferred License : RN License (NY License ONLY) Certifications : BLS (any provider is accepted) Must-Have : Epic experience and Oasis Assessment experience- Working knowledge of health care reimbursement systems, government regulations, and health industry accreditation standards- Experience with IVs and advanced wound care products- Strong interviewing, clinical assessment, and communication skills- Current NYS driver's license- Ability to work independently with minimal supervision Job Description : Develop care plans appropriate to the patients' and families' health care needs in collaboration with patient, family, physician, clinical operations team, and third-party payers- Obtain medical orders and approval of medical services- Complete the OASIS Assessment to accurately reflect clinical, functional, and social needs of the patient- Ensure the completion of all appropriate clinical records for compliance with state and federal legislation and seamless service delivery- Refer to appropriate health care systems and community agencies / resources- Communicate assessment and reassessment outcomes to the health care team to ensure appropriate follow-up- Display exemplary patience, courtesy, and flexibility when interacting with patients and staff- Resolve problems at the point of contact- Maintain appropriate productivity requirements- Adhere to Professional Home Visit Performance Standards
