About the Job
Community Health & Outreach Manager
Objective / Overview
CUHCC's Community Health Team is currently composed of three Community Health Workers who conduct outreach activities to connect community members to CUHCC deliver one-one CHW services, including health education, skill building self-management support and community connections.
The Community Health (CH) Manager oversees CUHCC's community outreach and partnerships, supervises the Community Health Worker team, manages the CHW internship program and coordinates health promotion programming.
The manager demonstrates experience with and understanding of Community Health Workers and outreach activities and with program delivery.
The Manager also provides CHW services including community outreach and a caseload of patients who receive one-on-one support with health education, home visits, self-management skill building and help with social needs.
The CH Manager provides leadership through training, mentoring, skills development monitoring, and programmatic oversight.
This role collaborates internally with medical, dental and behavioral health staff and externally with community-based organizations to coordinate CUHCC's participation in place-based outreach and community events.
The CH Manager demonstrates an advanced level of communication and interdisciplinary collaboration, professionalism, and systems-based thinking with an emphasis on whole-patient care model, education, community, and organizational engagement to help meet strategic goals.
Position Information
The position reports to the Director of Advancement. This is an in person position, 100% (40 hours per week) appointment at CUHCC.
The schedule will generally be 8 : 30am - 5 : 00pm, Monday - Friday, with the occasional evening / weekend requirements for events.
Travel locally may be required for events. The salary range for this position is $30.00 / hour ($62,400 annually) to $34.
00 / hour ($70,7200 annually), depending on education, experience and qualifications.
Essential Job Functions
Manage Community Health Worker Team (60%)
- Serve as primary liaison between CHW team, the Quality Director and CUHCC's services lines to integrate CHWs into the implementation of quality priorities
- Lead hiring, onboarding, ongoing professional development and performance management of community health workers
Provide daily supervision of CHWs : coaching, support and guidance in promoting adherence to the CUHCC's policies and care model, documentation of services and other evidence-based practices
- Develop and update CHW workflows related to CHW service delivery
- Generate and analyze reports (dashboard and other) to monitor productivity (number of visits, quality metrics, etc.) to support CUHCC's strategic priorities and to fulfill grant reporting.
- Manage CHW internship program : collaborate with universities and colleges, design and coordinate interns' experience, recruit and onboard interns, and evaluate the internship program.
Manage external partnerships and internal programs and classes (20%)
- Manage CUHCC's presence at select community events
- Develop, maintain and serve as a liaison for place-based partnerships with community-based entities (e.g., UMN Mobile Health Initiative, Minneapolis Public Housing Authority, schools) where CHWs conduct health education and outreach activities
- Coordinate and oversee implementation of health promotion classes / groups for CUHCC patients and quarterly Patient Advisory Conversations.
CHW Services (20%)
Deliver CHW services to a small caseload of patients : provide health education, assist with self-management skill building (e.
g., self blood pressure monitoring), identify social needs and help make community connections to address them.
CUHCC reserves the right to change or add duties consistent with the job classification.
Qualifications

Minimum / Essential Qualifications
- Bachelor's Degree plus at least two years of experience or a combination of related education and work experience to equal at least six years.
- Two or more years of experience managing / coordinating programs
- Two or more years of supervisor experience
- Related work experience including CHW service delivery, case management or related services in healthcare, social service, or community settings.
- MN CHW Certificate or achievement of MN CHW Certificate within 18 months of hire date
- Proficient in computer applications such as Word, Excel, and Powerpoint
Preferred Qualifications :
- Fluency in written and spoken either Spanish, or Somali, Hmong, Lao, or Vietnamese
- Experience in data reports and analysis
- Experience in a healthcare setting including experience with electronic health records
- Excellent communication skills
- Demonstration of strong teamwork and team building skills
