Description

: The Primary Care Health Home (PCHH) Health Navigator plays a crucial role in managing PCHH patients by utilizing a patient-centered approach to coordinate healthcare needs and wellness goals. The Health Navigator collaborates closely with healthcare providers, patients, and the PCHH team to develop and implement comprehensive plans addressing medical, behavioral, and social needs. Additionally, the Health Navigator manages care transitions, removes barriers to care, and facilitates effective communication among all members of the care team. The Health Navigator handles the clerical responsibilities of the program, including processing enrollment and discharge paperwork for the state, as well as managing multiple program spreadsheets and processing reports received from the state. The role requires a team-oriented individual with strong communication, problem-solving, and organizational skills.Education : ▪ Required : High School Diploma or Equivalent

Experience : ▪ No previous experience required ▪ Preferred : Previous experience with Electronic Health Records and / or 2-3 years experience working in a Family Practice out-patient setting

Skills : ▪ Strong organizational and multitasking abilities ▪ Excellent telephone and interpersonal communication skills ▪ Ability to multi-task and prioritize; strong organizational skills ▪ Strong knowledge of medical terminology ▪ Computer experience required ▪ Evaluation of patient, Care plan oversight and Care Coordination required.
