Hospice RN Case Manager

As a Hospice RN Case Manager, you will be responsible for providing direct patient care services, acting as the primary liaison between physician, patient / family, and hospice team, and empowering patient's caregivers through education and support.
You'll collaborate with an interdisciplinary group to provide comfort, peace, and dignity to patients and their families.
This role requires a compassionate and empathetic spirit, as well as an active RN license and 1+ year of nursing experience in hospice, home health, ICU, geriatric, or oncology.
