Job Title : Registered Nurse – Intensive Mobile Team (Full-Time)

About the Program
The Intensive Mobile Treatment (IMT) program is a trauma-informed, innovative mobile practice model designed to serve individuals whose needs are not met by traditional systems. IMT participants often interact with homeless services, criminal justice, and behavioral health systems. Our multi-disciplinary team provides comprehensive, flexible, and person-centered care focused on recovery, housing, harm reduction, and overall wellness.
Position Overview
The Registered Nurse (RN) functions as a primary practitioner within the IMT team, assessing and monitoring participants’ clinical health and response to treatment. Using a trauma-informed, recovery-focused approach, the RN collaborates with the team to support participants in achieving mental health, rehabilitation, and recovery goals.

Key Responsibilities
Develop, implement, and maintain IMT nursing standards, policies, and procedures.Conduct psychiatric and physical health assessments, especially for individuals experiencing homelessness or substance use.Collaborate with the team to create and monitor treatment plans.Order, interpret, and evaluate diagnostic tests and administer medications in conjunction with the psychiatrist.Provide trauma-informed individual and group health services, including administering IMs, vaccines, PPD tests, and phlebotomy.Monitor vital signs, medication side effects, and complete annual health assessments.Coordinate care with psychiatrists, physicians, hospitals, and community providers.Advocate for participants with collaterals, agencies, facilities, and family members.Provide consultation to team members on medical issues.Maintain accurate records, progress notes, and health data in AWARDS.Participate in 24-hour crisis intervention rotation.Qualifications
Current NYS RN license (required).Minimum 2 years of nursing experience.Minimum 2 years working with individuals with mental health conditions in a health or social services setting.Strong written, verbal, and interpersonal communication skills.Proficiency in Microsoft Office and ability to learn new electronic systems.Eligible for full and unconditional participation in Medicaid and Medicare.Comfortable providing care in non- traditional / community settings (streets, shelters, community spaces).Must be able to work in the community and use public transportation.Bilingual Spanish preferred.Must be fingerprinted and cleared by the NYS Justice Center.
