About The Position

Job Title : Discharge Planner / Case Manager Job Specialty : Social Services - Licensed Clinical Social Worker (LCSW) Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 16 : 30), with day and rotating weekend shifts Guaranteed Hours : Not specified Experience : Minimum years not specified, but prior experience is required License : Must have a DC License in hand.

Social worker must be eligible for LMSW / LGSW / LCSW and have passed the exam. Certifications : Basic Life Support (BLS), RN or LMSW / LGSW / LCSW Must-Have : - Experience in discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters.

• Experience with the following work setting skills : Cerner, Acute Hospital, Insurance Company, Long Term Acute Care / Rehab / Skilled Nursing.
• Regulatory skills involving CMS, Department of Health, HIPAA, and The Joint Commission.- Case Management / Utilization Review skills including Admission Criteria, Benefits Eligibility, Care Coordination, Discharge Planning, Needs Assessment / Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker’s comp case management.

Job Description : Perform discharge planning and case management.- Coordinate patient care and manage the plan of care.- Conduct needs assessments and order Durable Medical Equipment (DME) as needed.

• Review and ensure benefits eligibility and pre-certifications.- Handle prior authorizations and manage worker’s compensation cases.
• Work with geriatric, ICU, heart and vascular, general surgery, general medicine, and MICU patients.- Work every 5-6 weekends with one shift.
• Maintain a professional appearance; business casual attire required, no scrubs.- Ensure compliance with work policies, including mandatory flu vaccination without exemptions.

Additional Information : Orientation is 28 hours and is 50% non-billable.- Modules are to be completed pre-start and during orientation, varying by specialty.
