Job Description

Reports to : Director of Clinical Coaching & Patient Transformation

Start Date : Training begins December 2025 | Full caseload begins January 2026

Location : Optimal You Wellness Center – St Johns, Florida

Our care model is doctor-led and health coach–facilitated , integrating advanced diagnostics, lifestyle medicine, and mindset-based strategies to help patients achieve true transformation.

About the Role

We’re seeking a passionate and motivated Functional Medicine Health Coach to join our team. This individual will play a key role in guiding patients through personalized nutrition, lifestyle, and mindset changes that support long-term wellness.

You’ll collaborate closely with the Director of Clinical Coaching & Patient Transformation as well as the doctors, to ensure that patient care is coordinated, evidence-based, and deeply supportive.

Work Environment & Expectations

This is an in-office position that includes both in-person and tele-health coaching sessions . The ideal candidate will be :

  • Comfortable using and learning various digital platforms to manage care efficiently.
  • Professional in both presentation and communication—whether in person, on camera, or over the phone.
  • A role model for health and balance , living the same principles we teach our patients.
  • Adaptable in a fast-paced medical setting , while maintaining warmth, empathy, and composure.
  • Respectful and inclusive of all people regardless of race, gender, or background.
  • Confident working with patients of all ages—from infants to the elderly.
  • Excited to assist with community events, marketing, and networking to help grow our outreach.

Schedule & Structure

  • Full-Time | 40 hours per week (after training period)
  • Training Phase (December) :
  • Part-time onboarding and shadowing with our clinical team.
  • Exact training hours will be determined based on clinic needs and candidate availability.
  • Coaching Phase (January onward) :
  • Transition into a full patient coaching schedule.
  • Fridays : Reserved for administrative work, continuing education, and team collaboration.
  • Weekend / Evening Events : Participation may be required for occasional health fairs, workshops, and corporate events.
  • When events occur outside the regular workweek, the schedule will be adjusted to maintain a 40-hour total (for example, hours may be shifted earlier in the week).
  • Flexible Scheduling : Most sessions occur during standard business hours, with some flexibility to accommodate patient needs.

  • Education or training in functional medicine, nutrition, integrative health, or a related field
  • Strong skills in motivational interviewing and behavior change strategies
  • Passionate about root cause medicine and whole-person wellness
  • Excellent written and verbal communication skills
  • Organized, proactive, and comfortable with technology
  • Team-oriented, adaptable, and eager to learn
