Care Coordinator / Manager

Job Type : Part-Time / On-call (As Needed)
Position Overview :
Nu Grace Family Care is seeking compassionate and dedicated Home Health Aides (HHA) / Caregivers to provide high-quality, non-medical care to our clients.
The ideal candidate will assist clients with activities of daily living (ADLs) and instrumental activities of daily living (IADLs), ensuring that their needs are met in the comfort of their homes.
Our caregivers play a vital role in helping clients maintain their independence and improve their quality of life.
Key Responsibilities :
- Personal Care Assistance : Help clients with bathing, dressing, grooming, toileting, and other hygiene-related tasks.
- Mobility Support : Assist with walking, transferring, and repositioning clients as needed.
- Meal Preparation : Prepare nutritious meals, taking into consideration any dietary restrictions or preferences.
- Medication Reminders : Provide reminders for medications (no administering or handling of medications).
- Light Housekeeping : Perform basic cleaning tasks such as laundry, vacuuming, washing dishes, and maintaining a safe environment.
- Companionship : Engage clients in meaningful conversation and activities to reduce social isolation.
- Transportation Assistance : Accompany clients to medical appointments, grocery shopping, and other errands as needed (if driving is part of job duties).
- Monitor Client Condition : Report any changes in the client’s physical or emotional condition to the Care Coordinator or Manager.
- Follow Care Plans : Adhere to individualized care plans developed by the Care Manager or Care Coordinator.
Qualifications :
- Certification (if required) : Current HCA registration or CA-required caregiver training.
- Experience : Prior experience in home care, elderly care, or a related field preferred, but not required. Willing to train the right candidate.

- Ability to follow care plans and document tasks completed
- Basic housekeeping and cooking skills
- Physical Requirements :
- Ability to lift, transfer, or assist clients (up to 50 lbs)
- Ability to stand, walk, and kneel for extended periods
