The Mechanical Circulatory Support Program at the Smidt Heart Institute provides complete state-of-the-art care for patients with heart failure .
Devices that help the heart pump blood can save the lives of patients awaiting a donor transplant. For others, an implanted device can become a permanent treatment.
Led by Jaime Moriguchi, MD, Cedars-Sinai's program offers the most innovative cardiac technology, including left and right ventricular assist devices and Total Artificial Heart.
Patients can access novel technologies through clinical trials. Because the program is an integral part of the Heart Institute, patients have access to the nation's best cardiac programs and centers, including the Heart Transplant Program and Heart Failure Program .
Summary of Essential Duties :
The MCS coordinator will facilitate the care and education of patients and their families / caregivers from initial referral to the team, during the pre and post-operative periods, and continued ambulatory follow-up care ensuring continuity of care across the MCS continuum.
Participate in staff education, community and outreach programs regarding MCS to include but not limited to health care providers, EMS and outside Emergency Departments servicing our MCS patients.
Coordinate any additional testing and work-up needed prior to implantation under the guidance of physicians or advance practice providers.
Educate MCS candidates and their caregivers of pump operation, maintenance, expectation pre and post implantation for both patient and caregivers, risks, complications, benefits and lifestyle changes.
Coordinates inpatient care in various areas of the hospital including initial implantation completing vital implant form, intra-operative device monitoring / management during procedures requiring conscious sedation.
Participate in discharge process including assessment of readiness for discharge.
Ensure the patient's home environment is safe and secure; provide documentation to referring providers, EMS department, local hospital and electric company at discharge.
Work closely with financial coordinators and insurance company case managers to provide MCS information to assist in obtaining procedural and MCS supplies post implant coverage approval.
Qualifications
Qualifications : Education :
Education : BSN degree required
BSN degree required
License / Certifications :
Current California RN License required
BLS from the American Heart Association required
Specialty certification or willing to obtain within a year from start date
Experience :
A minimum of 3 years of acute care nursing experience required
At least 1 year of Critical Care experience required, preferably in a cardiac setting
Organ transplantation experience preferred
Demonstrated knowledge of case management required
Experience in data management preferred
Excellent organizational skills and ability to effectively coordinate all aspects of patient care required
