Responsibilities Coordinate and advocate for our patients care & create an appropriate discharge plan. Review patients charts, and meet with families and healthcare providers to establish the most suitable course of action.
Should remain in contact with patients and their families during the hospitalization stay. Qualifications Job Requirements : Current RN license in the state of Texas Registered Nurse with an Associates, Bachelor’s or Master’s Degree Minimum of three (3) years of clinical experience Preferred Job Requirements : Case Management certification Experience in utilization review, discharge planning, quality programs
